READS Scholarship: Now accepting admissions
Requirements:
• Application letter addressed to:
The Director of Scholarships
UNO-R, Bacolod City
• Two pieces 2×2 picture with name tag
• Baptismal Certificate (Original)
• Photocopy of parents’ latest Income Tax Return or BIR Exemption
(Combined income must not be more than ₱65,000)
• Photocopy of High School Report Card
(For College level, photocopy of latest grades with subject grades not lower than 80)
• Certificate of Good Moral Character
• Medical Certificate
• Family picture captioned with names of each family member
• Photo of the applicant’s house captioned with a brief description
• Recommendation from Parish Priest
• Barangay Clearance
• Photocopy of College Admission Test Result (if High School graduate)
Original copy of documents must be presented for verification
Applicants must be baptized and a practicing Catholic
Deadline of Application is on January 31, 2014
